NOTICE OF PRIVACY PRACTICES

THIS NOTICE DESCRIBES HOW MEDICAL AND DRUG AND ALCOHOL RELATED INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION.  PLEASE REVIEW IT CAREFULLY.

During your treatment at Seashore Family Services of N.J., doctors, nurses, and other caregivers may gather information about your medical history and your current health.  This notice explains how that information may be used, and shared with others.  It also explains your privacy rights regarding this kind of information under two Federal laws; the Health Insurance Portability and Accountability Act of 1996, (HIPPA), and the 42 U.S.C. 1320d et seq., 45 C.F.R part 160 & 164 and the Confidentiality Law, 42 C.F.R part 2.  The terms of this notice apply to health information created or received by Seashore Family Services of N.J.  We are required by law to: make sure that medical information that identifies you is kept private; give you this notice of our legal duties and privacy practices with respect to medical information about you; follow the terms of the notice that is currently in effect; and notify you in the event there is a breach of any unsecured protected health information about you.

Amy McGowan, M.S. L.A.C. N.C.C.Alcohol and Drug Counselor Intern
Amy began working for Seashore Family Services in March of 2017. She graduated from Monmouth University with her Master’s in Mental Health Counseling. She also holds a Bachelor’s of Arts Degree in Psychology from Monmouth University. Amy is a Nationally Certified Counselor with the goal of obtaining her Licensed Professional Counselor and Licensed Clinical Alcohol and Drug Counselor credentials. She is a member of Chi Sigma Iota, the International Honor Society for professional counselors. She is avid about working with the substance abuse population. Amy’s professional experience at Seashore involves working with both adolescents and adults of the mental health and substance abuse population. Amy’s previous professional experience includes working with adults with co-occurring illnesses in an inpatient facility, as well as working with women and children whom are survivors of domestic violence.

Barbara is the full-time secretary and provides office support in our Beaverson Office. She has more than thirty-five years of working experience in both legal and executive secretarial fields. Prior to joining SFSNJ, Barbara worked for the Arizona State Veterans Home in Phoenix, AZ as the Executive Secretary to the Administrator, where she worked on a daily basis with the resident Veterans and Executive staff. Barbara has a great deal of expertise and patience in dealing with people and is happy to be associated with SFSNJ.
